New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. Once you’ve scheduled your first appointment, the new patient forms will be emailed to you. You can either fill out the forms and submit them online or print them and complete them manually. Please submit or bring the completed forms, a list of all medications you are currently taking, and your insurance card to your appointment.

Billing

We require any applicable co-payment and/or past-due account balances at the time of service. We accept cash, checks, debit, and all major credit cards.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate. We accept insurance from most providers including:

• Aetna

• Medicare

• Cigna

• Tri-Care

• Humana

• and many more!

We recommend that you call your insurance company to verify that we participate with your plan.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments.

Supply Refill Requests

If you are in need of CPAP & Supply refills please submit your requests through the site, or by calling your preferred Sleeptopia location.
